Skill with that weapon (Unskilled bowmen are a liability - anyone can pick up a crossbow and aim in the right direction)
Being injured
Plate armour can make it harder to draw a bow
And finally... scenery and other characters can get in the way.

Dodging
Missiles can also be dodged. This simulates the extra difficulty of hitting a moving target.
The three main factors are:
How far a unit moved.
That unit's speed.
